NAIA District 28 consisted, until the end of the district qualification era, of NAIA schools in West Virginia. In general, as every school in West Virginia was a member of the West Virginia Intercollegiate Athletic Conference (1924-2013), there was no district playoff in years when the conference's regular season champion also won the conference tournament; if a different team won the tournament, a best-of-three series between the regular season and tournament champions ensued.

When the NAIA split into divisions in 1992, the teams in the district competed in Division I.
